A performance review is a formal assessment in which a manager evaluates an employee's work performance, identifies strengths and weaknesses and offers feedback.

During the monthly/ annual review for the period __, we noticed your performance was not up to the company’s performance standards.
However, we still sense potential in you and request you to work on your performance.
We would like you to work on your performance and bring it to acceptable standards. During this period, we advise you work with Mr./ Mrs. __ to get the guidance and help required in order to achieve this. Your performance will be regularly monitored over the next 90 days and the final decision will be taken based on the same.
